Application Checklist

 

Preschool Application process

Tours are given daily by appointment. You will be required to fill out a pre -application contact form. An appointment for the parent interview and student evaluation will be set up at this time.

To complete registration please return application forms along with all application fees. The following documents will be required listed below.

Elementary students will be given a recomendation form to be filled out by the administrator of students previous school. When we receive the recomendation by mail from previous school we will schedule a testing evaluation date for your student. An appointment for parent interview will be set up at that time and the testing results will be discussed during this meeting.

The application will be released upon students acceptance into our program. Please fill and return application along with all applicable fees. The following documents will be required listed below.

     . Birth Cirtificate

    . Updated Medical

   . Proof of Health coverage

  . Permanent record request form. ( Elementary School )

  . Payment in certified check, money order or credit card.
